Health Permit

In order to operate a new or remodeled food establishment in the City of Grand Prairie, the owner/operator must obtain a Health Permit (i.e., a “Food Service/Establishment Permit” or “Environmental Regulated Facility – Food Permit”) from the City of Grand Prairie Public Health and Environmental Quality Department.

On all plan reviews, the Public Health and Environmental Quality Department looks for:

  • Asbestos surveys
  • Potential cross-connections with the public water supply
  • Industrial waste sample points for the sanitary sewer
  • Grease traps, if necessary
  • Compliance with federal, state and local requirements

All applications must be presented with facility plans for review. Drawings must show a floor plan, fixture layout, room finish schedule, and plumbing riser diagram.

Apply

Step 1.Apply for Certificate of Occupancy

For a new business and/or change of ownership, a Certificate of Occupancy  Application must be submitted online to the Building Inspections Division. View the Certificate of Occupancy web page for information on how to apply.

Step 2.(If necessary) Apply for Commercial Building Permit

For new construction or remodeling, a Commercial Building Permit Application must be submitted online in addition to the Certificate of Occupancy. This includes all new construction and any type of remodeling, renovations, rehabs, retrofitting, demolitions, additions, etc. View the Commercial Building Permit web page for information on how to apply.

Step 3.Fill Out Solid Waste Verification Form

Fill out a Solid Waste Verification Form. You will be required to upload this document during the online application process for your Certificate of Occupancy and Health Permit.

Step 4.Apply for Health Permit

After you fill out the Certificate of Occupancy Application (Step 1), the Public Health and Environmental Quality Department will review the application and will send you a request to fill out a Health Permit Application(PDF, 422KB). The Health Permit Application must be submitted in person. (The Certificate of Occupancy and Building Permit applications must be submitted online through the Customer Self-Service portal.)

What happens next?

After completing the applications, you will see a list of required inspections within the Customer Self-Service portal. In addition to other inspections required for your Certificate of Occupancy, a pre-operational inspection by an Environmental Specialist is required prior to opening a food service establishment. City staff will contact you to schedule these inspections.

Permits will be issued when the facility is complete and in compliance with the rules and regulations from the City of Grand Prairie and the State of Texas.
