Property owners, Insurance companies or private investigators may request a fire report by submitting a Public Information Request online.

To request a medical report for dates of service prior to November 1st, 2024 visit submit a Medical Records Request online.

To request medical records for dates of service on or after November 1st, 2024, please download, complete, and submit the Patient Records Request Form(PDF, 129KB).
