Food Manager Certification

Requirements

At least one person in charge of a food establishment must possess a Certified Food Manager (CFM) Certificate obtained through a Texas Department of State Health Services (DSHS) accredited CFM training program.

In addition to the Food Manager Certification through DSHS, all individuals with a Certified Food Manager Certificate must obtain a Grand Prairie Food Manager Certification (GPFMC).

Certification Process

Step 1.Pass Required Exam for DSHS Certified Food Manager Certificate

The food manager certification is obtained by passing a Texas DSHS or ANSI-CFP approved food manager certification examination. The examination may be taken at any accredited examination provider or training program.

An official Certified Food Manager Certificate is issued to each candidate upon passing the examination. This certificate must be posted in an area that is conspicuous to the consumers. Certification is valid for five years.

Step 2.Apply for Grand Prairie Food Manager Certification

The City of Grand Prairie Food Manager Certification Application requires submitting the following information:

  • Your name, date of birth and contact information
  • Name of your employer and contact information
  • Copy of Driver's License or Government ID
  • Copy of the 5-year Food Manager Certification that was obtained through Texas DSHS
  • Payment of fee ($50 for Food Manager Certification, $10 for replacement card) - due after the Public Health and Environmental Quality Department reviews application and sends invoice.

What Happens Next?

After submitting application, a card/certificate will be issued within 48 hours. Food Manager Certification issued by the City of Grand Prairie are valid for 2 years from date of issuance.
